In Aquanaut, the protagonist has to find and kill the leader of an alien race that wants to destroy the Earth. This alien is intelligent and not easy to find, but mankind has managed to track him down to an underwater location in the Caribbean. The game is divided into three levels with different game mechanics.

The first level is a traditional horizontal scrolling shooter. The protagonist, equipped with scuba gear, has to make his way to the supposed position of the alien ship. The enemies are mutated underwater animals that are shot with the harpoon. There are also friendly creatures such as dolphins and mermaids (their kiss gives an extra life) and military crates containing extra equipment. Air is limited and must be refilled in certain parts of the level.

In the second level, the player has to investigate an underwater labyrinth. He has to find and pick up various objects that are used elsewhere to advance. This section is puzzle-oriented and the action focuses on obstacles to avoid and individual enemies.

Level three is also a maze in which you have to find objects (a decoder and keys) that you have to take to a final location. However, it is set on land (Atlantis, to be exact) and so air is no longer an issue. Combat takes a 'run and gun' approach in which the player shoots a lot of aliens and dodges their attacks. In this section, the game changes the screen when entering another room.
